What Is ECM?

Whole-Person Care, Coordinated for You

ECM is designed for Medi-Cal members with complex health and social needs. Instead of juggling many providers and programs on your own, you get one lead Care Coordinator.

Your coordinator works with your doctors, health plan, and community partners—often in person, where you live—so your care finally works together.

What ECM Includes

The Core Services of ECM

  • Outreach & Engagement

We reach out to eligible members in person and in the community to get started.

Setting Expectations

What Your Care Coordinator Does

ECM Is…

  • A dedicated person who knows your whole story
  • Help scheduling, rides, forms, and follow-ups
  • Someone who works with all of your providers
  • Support at home and in your community

ECM Is Not…

  • A replacement for your doctor
  • A cost to eligible Medi-Cal members
  • Emergency or 24/7 crisis care (call 911 or 988)
  • A one-time phone call
