Our Commitment

We aim to handle complaints fairly, respectfully, promptly, and without discrimination or retaliation.

Making a complaint will not negatively affect your ability to request or receive services from FOR Local Communities. We encourage you to tell us when something has gone wrong so that we can respond appropriately and identify opportunities to improve.

What Happens After a Complaint Is Submitted

When we receive a complaint, we will review the information provided and determine the appropriate next steps.

Depending on the nature of the concern, we may contact you for additional information, speak with relevant staff or service partners, review applicable records, policies, or procedures, and take appropriate corrective or follow-up action.

We aim to respond within a reasonable timeframe. More complex matters may require additional time to review.

Medi-Cal and CalAIM Concerns

If your complaint concerns Medi-Cal, CalAIM, Enhanced Care Management, Community Supports, or services administered through your Medi-Cal managed care plan, additional grievance or appeal rights may be available through your health plan or applicable government program.

FOR Local Communities can provide information about appropriate channels when applicable. Filing a complaint with us does not prevent you from exercising grievance, appeal, fair hearing, or other rights available under Medi-Cal or applicable law.

Confidentiality

Complaints will be handled with appropriate discretion. Information will generally be shared only with individuals who need it to review, investigate, respond to, or resolve the concern, or when disclosure is otherwise required or permitted by law.

No Retaliation

FOR Local Communities does not tolerate retaliation against anyone for raising a concern, submitting a complaint, participating in a complaint review, or exercising rights available under an applicable program or law.

Anonymous Complaints

You may raise a concern without identifying yourself. However, providing contact information may help us investigate the matter, obtain additional details, and communicate the outcome.

Our ability to investigate or resolve an anonymous complaint may be limited if sufficient information is not provided.

Urgent Health or Safety Concerns

This complaints process is not an emergency service.

If you or another person is experiencing an immediate medical or safety emergency, call 911 or contact the appropriate emergency service.

Continuous Improvement

Complaints and feedback help us understand where our services can be improved. We may use complaint information, while respecting applicable privacy requirements, to improve staff training, service delivery, accessibility, policies, procedures, and community programs.

Policy Updates

FOR Local Communities may update this Complaints Policy periodically to reflect changes in our services, procedures, or applicable requirements. The latest version will be published on our website with an updated revision date.
